QMHP-CS Caseworker – Mental Health Services (Hybrid)

Overview

We are seeking a compassionate and motivated Qualified Mental Health Professional – Community Services (QMHP-CS) to join our growing behavioral health team.

In this role, you will provide individualized support, counseling, and case management services to diverse populations including children, adolescents, adults, and individuals with developmental disabilities. Services are delivered across multiple settings, including outpatient clinic, community-based environments, and telehealth.

This position is ideal for professionals who are passionate about making a meaningful difference in the lives of individuals facing mental health challenges while promoting recovery, stability, and long-term wellness.

Key Responsibilities

  • Conduct comprehensive client assessments and assist in identifying behavioral health needs
  • Develop and implement individualized, recovery-focused treatment plans using evidence-based approaches
  • Provide individual and group skills training and supportive counseling
  • Support clients experiencing:
  • Emotional distress
  • Trauma-related symptoms
  • Grief and loss
  • Behavioral challenges
  • Substance use concerns
  • Crisis situations
  • Utilize effective crisis intervention and de-escalation techniques when needed
  • Coordinate care through case management services, including:
  • Intake support
  • Service linkage
  • Community resource coordination
  • Ongoing client monitoring
  • Deliver services via telehealth when appropriate
  • Maintain timely and accurate documentation in compliance with HIPAA and agency standards
  • Collaborate with multidisciplinary providers to support clients across the lifespan, including:
  • Children & adolescents
  • Individuals with Autism Spectrum Disorder
  • Geriatric populations
  • Clients with special education or chronic health needs
  • Assist in addressing social determinants of health through advocacy and resource connection
  • Stay current with clinical best practices and documentation standards

Qualifications & Experience

  • QMHP-CS designation (required)
  • Experience working in behavioral health, social services, or community mental health settings
  • Background supporting children, families, or individuals with developmental disabilities preferred
  • Knowledge of crisis intervention strategies
  • Familiarity with evidence-based approaches such as:
  • Motivational Interviewing
  • Cognitive Behavioral strategies
  • Behavioral interventions
  • Experience in outpatient or community-based care settings preferred
  • Telehealth experience is a plus
  • Strong documentation skills and understanding of HIPAA compliance
