Team Lead/ Manager (LPC/ LCSW/ LMFT)- Intensive Case Management

Description and Primary Purpose of Position:

The Team Lead position is responsible for providing administrative and clinical supervision, coordination, consultation, and facilitation of Intensive Case Management teams provide intensive comprehensive treatment/services to individuals with severe and persistent mental illness in the community where they live.

Duties and Responsibilities

Lead Treatment Team at least 1 day per week

Maintain a case load of at least 5 Consumers.

Review staff caseload and schedule to ensure they can maintain their required unit count.

Supervise all members of the team.

Assist with the Referrals.

Ensure that Consumers are scheduled for at least 4 visits per week.

Ensure all Failed Activities are completed within 24hrs by staff.

Participate in ASO Audit

Participate in CARF Review
