Reporting to the Medical Director, the Team Physician for Hospice & Palliative Care has the responsibility of assisting the Medical Director in the care and treatment of patients served by Gulfside Healthcare Services and provides physician direction and guidance to the Gulfside Healthcare Services program and staff to assure the maintenance of quality medical care for patients and families.
EDUCATION AND QUALIFICATIONS:
- MD or DO from an accredited medical school
- Licensed to practice medicine in the State of Florida
- American Heart Association accredited CPR Certification
- Knowledge and experience in palliative medical practice and symptom management preferred
- Board eligible or Board certified in a recognized field of the American Board of Medical Specialists
- Valid Florida driver’s license and automobile insurance as per Gulfside Healthcare Services policy
- Ability to travel to off-site locations
- Ability to make home/residence visits
ESSENTIAL JOB RESPONSIBILITIES:

- Represent Gulfside Healthcare Services professionally at all times through care delivered and/or services provided to all clients
- Comply with all state, federal and local government regulations, maintaining a strong position against fraud and abuse

- Observe Gulfside Healthcare Services health, safety and security practices
- Maintain the confidentiality of patients, families, colleagues and other sensitive situations within Gulfside Healthcare Services
- Participate in plan of hospice care for patients as directed by the Chief Medical Officer
- Certify and recertify patients based on clinical assessment of patient life expectancy of six months or less, meeting prognostic indicator criteria
- Provide patient visits as assigned by the Chief Medical Officer in patient homes, nursing homes, assisted living facilities or hospice houses
- Promote Gulfside Healthcare Services through participation in community and professional organizations
- Participate proactively in improving performance at the organizational, departmental and individual levels
- Improve own professional knowledge and skill level by attending hospice conferences and seminars
- Share expertise with co-workers both formally and informally
- Serve as a team physician for one or more of the interdisciplinary teams (IDG) as directed by the Medical Director.
- Participate in IDG meetings with Physician oversight as directed by the Medical Director.
- Validate patient/family plans of care in coordination with a patient’s attending physician
- Support IDG with clinical expertise and decisions on diagnosis and medical management of patient/family needs, focusing on continuity and comprehensiveness of care
- Consult to patient’s primary and attending physicians regarding integration of patient’s hospice care and primary or other care
- Assist with development of written guidelines for symptom control to Gulfside Healthcare Services committees
- On-going evaluation of patient appropriateness for hospice care
- Lead process for certification and recertification of patients to the IDG
- Participate in relevant Gulfside Healthcare Services committees
- Teach designated in-services to Gulfside Healthcare Services staff as needed
- Provide medical coverage for patients without attending physicians, or locate medical coverage for them, in completing medical record
- Represent Gulfside Healthcare Services at appropriate local, national and international forums
- Responsible for medical documentation of all patients seen who are cared for by Gulfside Healthcare Services
- Assist in improving quality of medical documentation by Gulfside Healthcare Services staff
- Contribute to Performance Improvement efforts of medical documentation by Gulfside Healthcare Services staff
- Complete physician billing forms following patient visits
- Promote Gulfside Healthcare Services programs to nursing homes, HMO’s, hospitals and other referral sources
- Care for patients in the Gulfside Healthcare Services inpatient facility
- Serve as designee in the absence of the Medical Director (if appointed/directed by Medical Director)
- In the absence of the Chief Medical Officer, the Team Physician would be responsible for emergency orders for Home Health patients should there be an urgent need and attempts to reach the patient’s attending physician are unsuccessful.
