Position Summary:

Under the supervision of Assisted Living Director, provide assistance to elders within the service district with transportation to and from medical appointments and assist during medical appointments. Assists elder with mobility in transportation and provides continuity of care between elder, elder caretaker and medical providers. Monitors basic health information such as; temperature, blood pressure, heart rate, oxygen levels and blood pressure. All Andahwod staff must adhere to mandated reporting laws. This is a grant funded position.


Essential Duties and Responsibilities:

  • Transports residents to and from their appointments using a program vehicle including operating a handicapped accessible van.
  • Transports or escort elder to and from residential home to vehicles and medical appointments to ensure proper mobility and safety of the elder.
  • Provides appointment reminders at least one day prior to medical appointment and assist with scheduling appointments as needed.
  • Assist with lifting, turning, moving and transferring elder to and from bed, seated position and/or vehicle, as needed and assist with mobility devices.
  • Record appointment time, date and location in the scheduling book as necessary.
  • Provide well checks on elders in the transportation program on a regular basis.
  • Assist elder with bowel and bladder functions; take to the bathroom, as needed during transport.
  • Explain all care to residents, their families and other medical personnel as necessary.
  • Set up and explain medical equipment to ensure elder is aware of the operation, maintenance and cleaning as prescribed.
  • Assist with picking up medication for elder which could include communication with the pharmacist and community health nurse.
  • Assist with home health care shopping needs as instructed by a medical provider.
  • Ensure recipients rights upholding their dignity, and treating the elder fairly, with kindness and respect.
  • Attend required training related to the Saginaw Chippewa Indian Tribal culture and providing culturally sensitive care.
  • Observe, make note of, and communicate (orally, in writing, via e-mail, or through documentation in computerized records) to the Assisted Living Director via direct supervision, any changes in behavior, capabilities, health status, demeanor, or expressed wishes.
  • Adhere to confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
  • Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
  • Must maintain strict confidentiality and present a positive, professional demeanor and image at all times.


Contacts/Purpose of Contacts:

  • Direct contact with residents, families, and guests to provide care; may encounter emotionally upset residents, family members, or visitors and must be able to handle difficult situations with patience, kindness, and respect.
  • Contact with health and behavioral health professionals, staff, and community resources to exchange information, for collaboration and consultation, and to provide education.
  • Contact with other tribal departments providing services to residents.


Minimum Qualifications:

  • High School Diploma or GED.
  • Certified Nursing Assistant.
  • Must be able to effectively communicate with elders and all members of the health care team and have good computer skills.
  • Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.

Desired Qualifications:

  • One year of direct care experience.
  • Experience with older adults.

License, Certification, or Special Requirements:

  • State of Michigan Certified Nursing Assistant.
  • Must be able to obtain HIPAA certification within five business days from date of hire.
  • Must be able to obtain BLS Certification within probationary period.
  • Must have a valid Michigan Driver’s License and must be able to obtain and maintain a Tribal Driver’s License and qualify for coverage by SCIT auto liability insurance carrier.
  • Native American preference shall apply to all positions.

Knowledge, Skills, and Abilities:

  • Knowledge of applicable federal, state, county and local laws, regulations, and requirements.
  • Knowledge of current principles, methods and procedures for the delivery of nursing procedures and quality of care.
  • Knowledge of prescribed and over the counter medication doses, side effects, and interactions for all residents.
  • Knowledge of long direct care nursing practices including patient care charts, patient histories, medical terminology, medical instruments, OSHA guidelines, and blood borne pathogens.
  • Skill in operating business computers and office machines, including Windows Office programs and E.H.R.
  • Ability to obtain accurate vital signs.
  • Ability to react quickly and make effective decisions in an emergency situation.
  • Ability to interact and maintain good working relationships with individuals of varying social and cultural backgrounds.
  • Ability to communicate efficiently and effectively both verbally and in writing.
  • Ability to multi-task and meet deadlines, and maintain confidentiality.
  • Ability to maintain quality, safety, and/or infection control standards.

Physical Demands:

  • Frequent use of hands, wrists, fingers associated with transporting elder populations and with computer equipment.
  • Required to stand, sit and walk for extended periods of time in all weather conditions.
  • Frequently required to bend, stoop, kneel, crouch or crawl.
  • Normal visual acuity, ability to talk and hear.
  • Occasionally lift and/or move up to 50 pounds.


Work Environment:

  • Elder homes, health care facilities and outside environment.
  • Potential risk of exposure to bodily fluids such as blood and saliva and/or the potential for needle sticks and/or skin puncture.
  • Latex may be used in the office which can produce allergic reactions.
  • Potential contact with chemicals and/or cleaning reagents utilized within the facility.
  • Employee may be exposed to unpleasant odors, blood-borne pathogens, hazardous material and infectious disease.
  • Extended hours as medical appointments may be scheduled will be required.
