The section of Transition Medicine (TM) in the Department of Medicine is recruiting a certified Physician to join our expanding primary care team. The Transition Medicine Clinic provides high-quality medical care to young adults and adults with Down syndrome, Spina Bifida, Autism, Cerebral Palsy, and other Intellectual and Developmental Disabilities. The candidate will provide outpatient services at Baylor St. Luke's Medical Center McNair Campus and the Center for Pursuit. Duties will include direct patient care, participation in the education of trainees, and participating in section meetings/conferences. Opportunities for research, quality improvement, and leadership activities will be strongly supported.

The Margaret M. and Albert B. Alkek Department of Medicine is the largest adult practice department at Baylor College of Medicine. Founded in 1953, it is one of the largest education programs in the United States with more than 700 trainees and a highly competitive specialty fellowship training programs, including seven federally funded research training grants. Summary

The Section of Transition Medicine at Baylor College of Medicine is seeking an outpatient Internal Medicine Physician to provide services at the Baylor Medicine McNair and Pursuit Center clinics. Ideal candidates are patient-centered and committed to excellence in providing quality care in an inpatient setting.

ESSENTIAL FUNCTIONS AND DUTIES

Faculty members are expected to demonstrate competencies in each of the following areas:

• Work with the primary care team to provide new or existing patients with primary medical care services within outpatient clinic setting.
• Develop a treatment plan for patients, order laboratory and radiology tests, and refer patients to specialists.
• Participate in all routine clinical meetings held with the immediate primary care team.
• Become familiar with Electronic Medical Record system, enter data into EMR, and develop a level of competency to document all patient encounters.
• Complete all documentation in a timely manner.
• Be an active member of the primary care team, participate in educational activities such as Grand Rounds, bi-weekly faculty and team meetings, and section retreats.
• Share clinical responsibilities within the primary care team including on-call duties for after hours, weekends, and holidays.
• Meet or exceed all quality indicators.
• Maintain required certifications and licensing, all appropriate certifications and licensing must be completed/renewed ahead of expiration dates.
• Maintain professionalism in relationships with students, faculty, ancillary and administrative staff.
• Adhere to departmental policies, procedures and objectives, ongoing quality improvement objectives and safety, environmental, and infection control standards.

Minimum Qualifications

Minimum Education:

• Medical Degree from an accredited United States medical school.

Minimum Job Experience:

• Residency & Fellowship training in Internal Medicine or Internal Medicine/Pediatrics

Minimum Certification/Licenses/Registration:

• License from Texas Medical Board certification.
• Board required to practice medicine in the state of Texas.
