We are seeking a compassionate, dependable individual to join our team in a unique and rewarding Transportation Certified Nursing Assistant (CNA) role. This position plays a vital part in ensuring our residents safely attend medical appointments while continuing to receive the high-quality, person-centered care they deserve.

Pay Range: $17.97-$22.91 per hour depending on qualifications and experience
Requirements:
Essential Duties:

Safely transport residents to and from medical appointments, hospitals, and other approved locations

Assist residents with transfers, mobility, and wheelchair transport

Monitor residents’ comfort and condition during transport

Communicate effectively with nursing staff regarding resident needs and appointment updates

Provide hands-on care as needed before and after appointments

Support residents in maintaining independence and dignity

Follow care plans and instructions from licensed nursing personnel

Maintain a team-oriented, compassionate approach to patient care

Ensure vehicle safety checks and cleanliness standards are met
Perform other functions as required to fulfill the expectations of the CNA.

Provides support of daily living activities by assisting with serving meals, feeding patients as necessary and ambulating, transferring, turning, and positioning patients; and providing fresh water and nourishment between meals; dressing; oral care; toileting; and general hygiene needs.

Maintains patient stability by checking vital signs and weight; recording intake and output information.

Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ call lights and requests; and reporting observations of the patient to nursing supervisor.

Documents actions by completing forms, reports, logs, and records.

Ensures residents/patients are ready, clean and well-groomed and on time for special tasks, such as visits, physician appointments, etc.

Ensures that any resident/patient change of condition is reported promptly to the licensed nursing professional.

Assists in admitting and discharging patients/resident, including inventory of all possessions, etc.

Maintains work operations by following policies and procedures and regulations.

Completes daily and purposeful rounding that ensures patient/resident environment is clean and free of hazards. Assists with cleaning and organizing patient’s/resident’s room and property to ensure all patient/resident needs are being met.

Updates job knowledge by participating in educational opportunities, reading professional publications, participating in professional organizations, and maintaining licensure.
Minimum Required Education/Experience: High school diploma required. Certification with accredited training course through the State of Colorado for Certified Nurse Assistant. Valid Colorado Driver's License. Previous work experience is preferred.
Pre-Employment Knowledge, Abilities and Skills:
- Basic knowledge of anatomy, physiology, and medical terminology.
- Knowledge of relevant nursing care.
- Friendly and professional bedside manner.
- Effective oral, written, and reading communication skills.
- Ability to establish and maintain effective relationships with the public, other agencies, employees and administration.
Note: An acceptable combination of education, training and experience that provides the above knowledge, abilities and skills may be substituted.
Necessary Special Requirements: Must obtain annual flu vaccination and any job-specific immunizations. Required to submit to tuberculosis test/screening.
Licenses or Certificates: Valid Colorado Driver's License. Certified Nurse Assistant license is required. Basic Life Support (BLS) certification is required.
Unusual Demands: Work is subject to recurring and inflexible deadlines and frequent interruptions. A task list of specific duties may be added to a job description providing more detailed instruction for specific tasks.
