Transportation Aide/CNA

Job Summary:
Canopy Care Solutions dba Heather Gardens, previously known as Scandinavian Homes (an affiliate of PACE-RI) is seeking a dependable, compassionate CNA to join our team. This role is designed for a highly skilled professional who can work with minimal supervision to provide high-quality care within our medical and social environment. If you take pride in relating well to residents and promoting their independence through consistent, quality support, you will fit right in here.

Your core responsibilities will focus on assisting residents with Activities of Daily Living (ADLs), personal care, and rehabilitation skills. Beyond standard CNA duties, you will play a key role in coordination by managing the appointment book and safely transporting residents to outings, banking, and medical visits in our company van (including the use of wheelchair "tie-downs"). You’ll also ensure medical forms are handled correctly between providers and our nursing team, keeping our residents’ care seamless and organized.

This is a 32-hour-per-week position with a steady Monday through Friday schedule—no late nights or weekends required. We offer shifts from 8:00 AM – 2:30 PM or 8:30 AM – 3:00 PM, making it an ideal fit for someone looking for a professional healthcare role that still allows for a great work-life balance.
