Requirements
Valid Driver’s License with no restrictions. Must be at least 21 years old. Must be eligible to be listed as insured on the commercial auto policy.
Physical Requirements
- Good visual acuity and ability to communicate.
- Ability to lift a minimum of 30 pounds and ability push/pull a minimum of 30 pounds, which includes the lifting, pushing and/or pulling of medical supplies and equipment and the transferring and repositioning of patients.
- Reasonable assistance may be requested when lifting, pushing, and/or pulling are undertaken which exceeds these minimum requirements.
- Ability to withstand prolonged standing and walking.
- Ability to reach, stoop, bend, kneel, and crouch are required for patient care functions and in setting up and monitoring equipment
Description
- Must follow all current safety and traffic laws in North Carolina.
- Ensure that vehicle has:
- Regular and routine maintenance
- Report all repair and maintenance needs promptly.
- Maintain transportation logs and make sure that they are totaled daily.
- Respect the rights of residents at all times.
- Serve as a liaison to outside agencies and treat others with courtesy and respect, making sure to maintain a professional image at all times.
- Keep vehicles clean from trash and keep exterior maintained to the best of ones ability.
- Enforce facility policies while in company vehicles.
- Ensure that residents do not smoke or eat in company vehicles.
- Maintain schedule of all resident appointments, making sure that adequate time is allotted for appointments and transportation to and from – including notifying Administration/SIC if additional resources will be needed for making all scheduled appointments 24 hours prior to scheduling conflict when possible.
- Ensure that all documentation and photocopies are completed for appointments on a weekly basis including, but not limited to: Medication Reconciliation with Facility Notes, Physician Orders, Insurance Information, and Contact Information for persons with a guardian.
- Make sure that all residents are clean and neatly dressed for appointments, by giving patient care staff adequate notice of scheduled appointments.
- Ensure that residents have had all relevant care activities performed prior to leaving facility, such as medication administration, meals, and toileting when appropriate.
- Ensure that transportation is scheduled for residents receiving transportation from outside resources such as Veterans or those receiving transportation by their mental health provider.
- Attend appointments with residents requiring help, if permission is granted from them or their legal representative.
- Attend meetings, classes, and other scheduled activities required for job role.
- Perform other job related duties as delegated by the supervisor and/or Administration.
