Job Description for Transportation Coordinator
Department: CGP PACE
Position reports to: PACE Center Manager
Position supervises: Drivers
FLSA Status: Exempt

Position Summary:
The Transportation Coordinator plans, organizes, and ensures effective transportation services for participants of the Care Guide Partners PACE program.
Position Duties & Responsibilities:- Plans and coordinates all scheduling activities related to transportation for Care Guide Partners PACE participants. Partners closely with individuals scheduling appointments and may participate in scheduling appointments as needed.
- Effectively communicates with Interdisciplinary Team, participants, and family members to notify and confirm transportation needs; ensures pick-up and delivery of participants are within acceptable time frames.
- Reviews scheduling and routes to optimize travel time and mileage.
- Supervises the activities of the Drivers on a daily basis. Receives reports regarding participant status and communicates concerns to Interdisciplinary Team.
- Ensures drive times and participants’ time in vehicles during transit are within regulatory requirements.
- Oversees competency requirements and training of Drivers, including body mechanics and use of mechanical assistance to transport participants.
- Coordinate all outside service provider transportation arrangements as needed, including proper vehicle maintenance and condition.
- Assures all necessary program records are maintained according to policy. Oversees vehicle maintenance program, including preventive maintenance and repairs.
- Participate as a member of the Interdisciplinary Team: attend staff meetings, take part in participant care planning, and communicate any reported changes in participant condition to team members.
- May be occasionally required to serve as back-up Driver.
- Other projects and duties as assigned.
Position Qualifications:- High school diploma, or equivalent.
- Valid Driver’s license and an acceptable driving record required.
- Minimum 2 years supervisory experience.
- Minimum 1 year of experience working with a frail or elderly population or appropriately trained upon hire. Previous experience transporting individuals with special mobility needs strongly preferred.
- Service excellence oriented with the ability to work well under pressure.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and multi-task with high level of quality and productivity.
- Strong analytical and problem-solving skills.
- Flexibility to travel to sites within the PACE service area.
- Must maintain CPR Certification and participate in first aid and defensive driving training.
