Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. You will work collaboratively with healthcare professionals to ensure the well-being of patients and their families while adhering to best practices and regulatory standards.

Responsibilities

• Deliver direct patient care, including administering medications, performing treatments, and monitoring vital signs.
• Assist in the development and implementation of individualized care plans.
• Conduct patient assessments and document medical histories accurately.
• Provide education and support to patients and their families regarding health management and disease prevention.
• Collaborate with interdisciplinary teams to coordinate patient care effectively.
• Maintain accurate medical records in compliance with HIPAA regulations.
• Perform procedures such as IV insertion, catheterization, and wound care as needed.
• Participate in quality improvement initiatives and adhere to infection control protocols.
• Engage in triage and emergency response situations as required.

Qualifications

• Current LPN license in good standing.
• Experience in critical care, long-term care, or assisted living preferred.
• Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is a plus.
• Knowledge of ICD-9/ICD-10 coding and medical terminology.
• Strong understanding of patient assessment techniques and basic life support protocols.
• Experience working with diverse populations, including those with developmental disabilities or dementia care needs.
• Excellent communication skills and the ability to work effectively within a team environment.
• Proficiency in performing phlebotomy, medication administration, and other clinical procedures.
