American Traveler is seeking a Home Health Physical Therapist for a 13-week travel contract covering multiple counties in South Carolina, requiring at least 1 year of experience and a valid PT license.

Home health setting serving patients across Clarendon, Sumter, Lee, Darlington, and Florence counties in South Carolina

Shift hours are 8:00 AM – 5:00 PM

Minimum expectation of 20 in-home patient care units per week

Position is unit-based; visit types include routine PT visits, start of care, evaluations, reassessments, supervisory visits, OASIS discharge, recertification, and resumption visits

Mileage reimbursement is provided starting from the closest branch location or first patient's home

Minimum 1 year of physical therapy experience required

Graduate of an accredited Physical Therapy program required

Current PT license required; license required if applicable

BLS certification required (AHA only)

Must possess a valid driver's license and reliable personal vehicle for travel to patient homes

Strong written and verbal communication skills required

Must be knowledgeable of state therapy laws and home health regulatory requirements, including CMS Conditions of Participation and JCAHO standards

Ability to perform moderate physical activity including frequent walking, standing, bending, and lifting up to 50 lbs
-
Full range of motion and ability to perform strenuous movements during patient care required

Must be proficient with computer use and standard office equipment

Strong organizational, critical thinking, and independent judgment skills required

Responsibilities include providing PT services per physician orders across in-home settings, including evaluations, treatments, reassessments, and OASIS documentation
-
Works closely with a Team Clinical Manager and collaborates with patients, family members, agency personnel, and physicians
-
Must be familiar with growth and development principles across the lifespan and able to assess age-specific patient needs

Orientation is limited to no more than 2 weeks
-
RTO is limited; requests should not exceed 2 consecutive days

Candidates must be able to work independently and manage their own schedule in a largely autonomous, field-based environment
-
Travel to rural and isolated areas is expected; safety awareness in varied home environments is essential
-
Department dress code applies; a neat and professional appearance is required
