American Traveler is seeking a Physical Therapist Assistant for a home health position in the greater Charlotte, NC area, requiring at least 1 year of home health experience and HCHB proficiency.

Home health setting covering Northern Charlotte and surrounding areas including Huntersville, Cornelius, Davidson, Morrisville, and Salisbury

Caseload of approximately 6 patient visits per day with an average driving distance of 60–80 miles per day; mileage is reimbursed weekly
-
Patient population includes those with chronic diseases, orthopedic conditions (total hips/knees), falls, generalized weakness, CHF, COPD, and occasional stroke diagnoses
-
Uses HCHB (Homecare Homebase) as the primary EMR — experience required
-
Epic Rover used in a view-only capacity; Focura used for uploading discharge instructions and wound documentation
-
12-hour day shifts, 8:00 AM–8:00 PM, 36 hours per week
-
No weekend rotation, no on-call requirements, and no major holiday coverage required
-
13-week schedule cycle; RTO subject to manager approval

Active NC PTA license required (license required if applicable)

Current BLS certification required

Valid Driver's License and proof of auto insurance required

Minimum 1 year of home health experience required, with most recent experience within the last year

Proficiency in HCHB (Homecare Homebase) required

Skills in gait training, balance training, non-pharmaceutical pain management, and care coordination required

Ability to recognize signs and symptoms that may prevent hospital readmissions
-
Strong communication skills for collaboration with supervising PT and clinical team

Last 4 digits of SSN and full date of birth required for consideration

Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN required for consideration

First-time travelers will be considered if they have solid home health experience
-
Candidates must reside 50+ miles from the facility's home unit to be eligible — local candidates are not accepted

Candidates who have worked a permanent position at an AHSE facility must have been separated for at least 1 year before being eligible

Orientation consists of 5 days of facility/unit onboarding (NEO)

Supported by a team including a Clinical Manager, Clinical Supervisor, Scheduler, and peers
