About the Role

An acute care hospital in Santa Rosa, CA is seeking an experienced Registered Nurse Case Manager for a 13-week travel assignment.
This role is ideal for an RN with a strong background in care coordination, discharge planning, and utilization review within a fast-paced acute care environment.

You’ll work closely with an interdisciplinary team — including social services, therapy, and medical staff — to ensure safe, efficient, and patient-centered discharge planning.

Key Responsibilities

• Perform comprehensive case management and care coordination for acute care patients
• Conduct discharge planning, arrange DME, SNF, and ambulance transport independently
• Collaborate with physicians, social workers, and interdisciplinary teams to develop individualized care plans
• Facilitate family meetings and participate in treatment planning
• Ensure compliance with CMS, HIPAA, and The Joint Commission standards
• Utilize Epic for charting and documentation
• Work every other weekend as part of department coverage rotation

Requirements

License: Active California RN license
Experience: Minimum 2 years of acute hospital case management experience within the past 2 years
Certifications: BLS (required)
Charting System: Epic (required)
Travel Experience: Preferred (first-time travelers may be screened if qualified)
Preferred Experience:

• Familiarity with HMOs, IPAs, and insurance-based case management
• Knowledge of InterQual criteria, discharge planning, and benefits coordination
• Experience in behavioral health, ICU, ED, or med-surg case management

Skills & Competencies

• Strong knowledge of care coordination, discharge planning, and benefits eligibility
• Ability to independently manage complex discharges and resource referrals
• Competence with CMS, DRG, and regulatory compliance
• Excellent communication, organization, and time-management skills
• Collaborative and patient-centered approach to interdisciplinary care

Unit Information

Department: Case Management
Shift: 5x8-Hour Days (08:00 – 16:30)
Weekend Coverage: Two weekends every six weeks
Float Requirement: Yes – to meet department needs
Orientation: 40 hours
Patient Population: Adults, Geriatrics
Interdisciplinary Support: IV therapy, social services, PT/OT, respiratory therapy, radiology, transportation, unit secretary
Documentation: Computerized charting (Epic)

Travel Contract Details

Length: 13 weeks
Start Date: 12/1/2025
Pay: $3,400 per week (based on 40 hours)
Overtime: $110.50/hr after 40 hours/week (1.3x hourly pay)
Holiday Pay: $110.50/hr
Weekend Coverage: Required – 2 weekends every 6 weeks
