Description

American Traveler is seeking an experienced RN for a Case Management position in Vicksburg, MS, requiring a minimum of 3 years of acute hospital case management experience and recent hands-on experience in both discharge planning and utilization review.

Details

  • Day shift position, 8:00 AM – 4:30 PM, with rotating weekend shifts
  • Acute care hospital Case Management unit
  • Caseload involves discharge planning to home health, SNF, LTAC, and IRF settings, as well as utilization review for medical necessity
  • Utilization review performed using MCG criteria
  • EMR: Cerner; MorCare used for CM documentation; CarePort used as the referral system

Requirements

  • Active MS or Compact RN license required
  • Current BLS certification required; no certifications may expire within the first 30 days of the assignment start date
  • Minimum 3 years of acute hospital case management experience required
  • Recent experience in both discharge planning and utilization review in a hospital setting required
  • Candidates must reside more than 50 miles from the facility to qualify for this position
  • No gaps in work history of 90 days or more within the past 12 months
  • Valid driver's license required for consideration

Additional Information

  • This is a strictly acute hospital case management role — outpatient, home health, hospice, charge nurse, or staff nurse backgrounds do not meet the requirements for this position
  • Must be prepared to function independently as an acute hospital case manager from day two of the assignment
  • Responsibilities include facilitating appropriate discharge plans, performing utilization review for medical necessity, and issuing patient choice letters and Important Message from Medicare (IMM) notices per CMS Conditions of Participation
  • Candidates who have worked at any CHS facility as permanent staff within the past 2 years are not eligible for this position
  • RTO requests must be for less than one week in duration
