American Traveler is seeking a licensed RN for a Case Management position in Alaska, requiring at least 1 year of case management experience and an active Alaska RN license.

Day shift schedule

Outpatient clinic setting focused on care coordination and patient case management

Patient population includes those requiring referrals to surgery and specialty clinics, with a focus on maximizing healthcare outcomes through interdisciplinary collaboration

Some direct nursing duties in the clinic are required in addition to case management responsibilities

Active, unrestricted Alaska RN license required at time of consideration

Current BLS certification required

Minimum of 1 year of case management experience required

Graduate of an accredited nursing school required; Bachelor's degree preferred

Flu vaccination required

Strong assessment, critical thinking, and problem-solving skills

Effective oral and written communication skills

Ability to multi-task and work independently in a fast-paced environment

Collaborates with physicians and interdisciplinary team members to establish individualized care goals and coordinate patient referrals to surgery and specialty clinics

Serves as the primary clinical contact for care coordination, proactively identifying and addressing delays or obstacles to patient care

Performs staff nursing duties including assessment, documentation, patient education, and medication administration as needed

Maintains annual competencies including safety and infection control requirements

Participates in department and clinic team improvement efforts

Start of assignment requires fingerprint card processing — travelers may not begin until at least 3 weeks after fingerprint cards are received at the facility's administrative office in Juneau
