Description

American Traveler is seeking an experienced RN Case Manager for an acute care hospital in Rapid City, SD, requiring Epic proficiency and prior acute care case management experience.

Details

  • Position is based in the Case Management unit of an acute care hospital in Rapid City, SD
  • Case Managers provide care for patients across all acute care departments
  • Caseload ratio of 1 CM to 16–20 patients
  • Travelers typically remain in the same area throughout their contract and are not required to float
  • Schedule is 8-hour day shifts, 0800–1630
  • Weekend rotation of 2 weekends per 13-week assignment; weekend team includes 3 RN Case Managers, 1 Social Worker, 1 Lead Case Manager, 1 Case Manager Assistant, and 1 Utilization Review Nurse
  • No on-call requirements
  • Holiday coverage expected for 2 out of 4 major holidays
  • EMR: Epic (experience required)

Requirements

  • Active RN license in South Dakota or a Compact RN license currently in hand
  • Current BLS certification required
  • Minimum of 2 years of RN experience required for first-time travelers; otherwise, 1.5 years of general nursing experience required
  • Prior acute care case management experience required, including discharge planning, progression of care, and family care plan discussions
  • Epic EMR proficiency required
  • Two professional references dated within the past year required for consideration
  • DOB and SSN required for consideration
  • Candidates must reside more than 100 miles from any Monument Health facility

Additional Information

  • Responsibilities include discharge planning, progression of care coordination, and meeting with patients and families to discuss plans of care
  • Supported by a team of Case Management Assistants, Lead Case Managers, Social Workers, Utilization Review RNs, and Hospitalists
  • Orientation is approximately 1.5 weeks, including an Epic training class
  • RTO of up to 7 days allowed; a full week off requires a contract extension
  • Scrub color is Navy Blue or professional attire
  • Free parking is available on-site
  • First-time travelers are considered with 2 or more years of experience
  • Former permanent staff must wait 1 full year before returning as a traveler
