Description

American Traveler is seeking an experienced RN Case Manager for an inpatient case management position requiring strong experience with complex discharge planning and social determinants of health.

Details

• Inpatient case management unit within a hospital setting
• Average daily caseload of 15 cases
• Cases involve complex discharge planning including coordination of home care, placement, and social wrap-around services for safe discharges
• Monday through Friday schedule, 8:00 AM – 4:30 PM (5x8-hour shifts)
• Holiday coverage required as scheduled

Requirements

• Active MD or Compact RN license required
• Inpatient case management experience required
• Experience with complex cases including locating family members, friends, and neighbors for patient support
• Willingness and ability to work on guardianships, capacity evaluations, certificates of competency, and collaboration with legal teams, including attending court hearings with patients
• Social work background preferred
