Description

American Traveler is seeking an experienced Home Health RN for a 13-week travel contract in Vermont, requiring a minimum of 2 years of experience and OASIS documentation proficiency.

Details

• Home health setting providing care to patients in their place of residence
• Serves as Case Manager for an assigned patient caseload
• Patient population includes complex cases requiring high-tech skills such as IV therapy, wound VAC, and complex wound care
• Schedule is Monday through Friday, 8am to 4pm, with some weekend and on-call requirements
• Uses McKesson EMR for documentation and visit management

Requirements

• Active VT RN license or a compact RN license required
• Minimum of 2 years of RN experience required
• OASIS documentation experience required
• Proficiency with high-tech nursing skills including IV use and maintenance, wound VAC, and complex wound care
• DOB and SSN required for consideration

Additional Information

• Conducts comprehensive assessments using the nursing process to develop and implement individualized plans of care
• Completes and maintains accurate OASIS documentation in accordance with agency, state, and federal requirements
• Collaborates with and delegates tasks to LPNs within their scope of practice
• Coordinates interdisciplinary care with physicians, physical therapists, social workers, and other team members
• Participates in IDT meetings and case conferences to support individualized patient care planning
• Performs patient and caregiver education on topics including medication side effects, treatment options, and discharge instructions
• Coordinates patient discharges and transfers with appropriate documentation and communication
• Participates in monthly peer review audits
