American Traveler is seeking an experienced RN for a Home Health and Hospice position in Montana, requiring a minimum of 2 years of relevant experience and an active MT or compact RN license.

Home health and hospice setting, with patient care provided in residential settings and at an in-patient Hospice Center
-
Serves a patient population requiring skilled nursing, case management, symptom management, and end-of-life care

Clinical skills utilized include venipuncture, catheterization, wound care, and parenteral therapy

Typical in-home RN schedule is Monday–Friday, 8am–5pm; typical in-patient RN schedule consists of 3 rotating 12-hour shifts

On-call rotation participation is required as needed to meet patient care needs

Active RN license in the State of Montana (MT license or compact RN license currently in hand)

Minimum 2 years of recent acute care, home health, or hospice nursing experience

Valid driver's license, appropriate auto insurance, and reliable personal transportation required

BSN preferred

Proficiency in venipuncture, catheterization, wound care, and/or parenteral therapy preferred

DOB and SSN required for consideration

Provide skilled nursing care and case management to patients and families using an inter-disciplinary team approach

Responsible for admitting patients onto services and coordinating care across disciplines, community health, and social agencies

Oversees and evaluates care provided by Home Health/Hospice Aides and CNAs for assigned patients

Maintains accurate patient records in compliance with agency standards and OASIS requirements

Participates in quality improvement activities and assigned in-service training
