Description

American Traveler is seeking a Home Health RN for a day shift position in the West Portland, OR area, requiring skilled home health experience and an active Oregon RN license.

Details

• Home health setting serving West Portland, OR and surrounding areas
• Visits up to 6 patients per day with a standard productivity expectation of 30 points
• 8-hour day shifts (5x8); 4x10 and 3x12 schedules cannot be accommodated
• No weekend or on-call requirements
• Floating between all facilities within Oregon is required and refusal may result in termination of assignment
• Charting via Epic Remote Client for Home Health (on laptop) and Rover (phone app); OASIS documentation required
• OASIS experience is required for this position

Requirements

• Active Oregon (OR) RN license required
• Current BLS certification required
• Minimum 1 year of skilled home health experience required
• Epic EMR experience required
• OASIS experience required
• Personal reliable transportation required
• Full SSN and DOB required for consideration
• Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
• Primary COVID-19 vaccine series plus at least one booster required

Additional Information

• Provide skilled nursing care to home health patients across West Portland and surrounding areas
• The system groups clients to minimize drive and windshield time; occasional daily radius of up to approximately 60 miles (60–90 minutes total drive time) is possible
• Business casual attire required; scrubs are allowed but not required, and jeans are not permitted
• Local candidates are accepted; candidates living within a 50–75 mile radius are considered local and are eligible at the same rate
• Former permanent employees of this health system must wait at least 3 months before returning as a traveler, and must not reside within 50 miles of the facility
• RTO is expected to be minimal; a few days may be approved but significant RTO requests are discouraged
