Responsibilities

Provide clerical support for the Case Management Department, including processing authorizations and prior authorizations, post-acute referrals, and delivering Medicaid notices
Coordinate discharge planning, referrals to rehab and long-term care, and arrange DME and home services
Document benefits eligibility and care coordination activity in EPIC
Facilitate communication among internal teams and external providers to ensure timely discharges and appropriate referrals
Support referrals to interpretation services, social services, pharmacy, and transportation as needed
Maintain accurate documentation for 18–24 patient caseloads (weekends may approach 30) in a fast-paced environment

Requirements

Minimum experience: 1 year in a similar clerical or health care coordination role; first-timers accepted
EPIC charting experience required
Certifications/Skills: EPIC charting proficiency; strong organizational, multi-tasking, and communication abilities
Licenses: Washington license not required for this role
Education: Not specified
Additional: Ability to work Mon–Fri 07:30–16:00 (may flex to 08:00–16:30); unit focus in Case Management / Care Coordination; on-site eligibility in Everett, WA
References: provide a minimum of 2 supervisory references covering 1 year of employment
