Responsibilities
- Provide direct patient care across multiple healthcare environments, including hospitals, nursing homes, assisted living facilities, and home care settings.
- Assist with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts and other assistive devices.
- Monitor vital signs, document patient information accurately using Electronic Health Record (EHR) systems such as Epic or Cerner, and report changes to medical staff promptly.
- Support infection control practices through aseptic techniques and proper medical documentation to ensure patient safety and compliance with HIPAA regulations.
- Administer medications and perform basic medical procedures such as catheter care, phlebotomy, and wound care under supervision.
- Collaborate with multidisciplinary teams including physicians, nurses, therapists, and social workers to coordinate comprehensive patient care plans.
- Assist with specialized care needs including dementia, Alzheimer's, memory care, behavioral health, pediatric care, and trauma-related services like Level I & II trauma centers or ICU environments.
Qualifications
- Valid CNA certification with current licensure in applicable states.
- Experience working in diverse settings such as hospitals (including emergency medicine or ICU), long-term care (nursing homes or assisted living), behavioral health units, or home health.
- Proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate medical documentation.
- Knowledge of physiology, anatomy, medical terminology, medication administration, vital signs monitoring and infection control protocols.
- Experience with specialized procedures including phlebotomy and use of medical equipment like Hoyer lifts or catheters.
- Ability to adapt quickly to different environments while maintaining high standards of patient service and safety.
