About the Agency: MedAssist empowers patients facing medical challenges to thrive by addressing their needs with comprehensive access to services through care management. Our Agency is expanding and we are looking to add some great people to our team!

Position Overview: As a Visiting Provider, you will be traveling to patients homes to complete visits with them. The main population we serve are older patients, with Medicare insurance. Our ultimate goal is to support our patients as they age in place by encouraging preventive screenings and medical follow-up, thereby reducing hospital/rehab recidivism. This position reports to the Intake Department Manager.

General Responsibilities (including, but not limited to):

• Travel to patients’ homes and provide them with healthcare services.
• Monitor your patients’ conditions and document your assessment in the EMR.
• Listen to the concerns of your patients, their family members, caregivers, and actively update your care plan with them.
• Complete an initial visit with the patient, assessing their needs.
• Collaborate with other healthcare professionals to provide patients with the best possible care based on the patient’s specific chronic conditions.
• Record visit notes, upload scripts and any other pertinent information in the EHR.
• Attend and participate in patient case conferences and required meetings.
• Follow up on medical supply orders (e.g., wound care items, incontinence supplies) processed to ensure they are expedited timely.

Requirements and Qualifications:

• Current unrestricted nurse practitioner licensure in the State of New York.
• Ability to travel to patients homes to provide care.
• Well-versed in EHR.
• Reliable & dependable.
• Bilingual preferred.
