The LVN performs services requiring technical and manual skills and obtains basic patient data, i.e., vital signs, weight, medications taken, I&O, assist in managing wounds, bowel, and bladder care management etc., participates in planning, executes interventions in accordance with the care plan or treatment plan, and contributes to evaluation of individualized interventions related to the care plan or treatment plan.

The LVN assumes the following responsibilities:
1. This position requires flexibility in schedule and assignments, as this is a 24/7 hospital.
2. Provides direct patient care to Surgical Specialty patients in accordance with hospital policies and practice standards and guidelines.
3. Obtains/collects specimens as ordered.
4. Participates in Surgical Specialty performance measure activities such as, but not limited to, hand hygiene, universal protocol, and access to clinics.
5. Administers medications and treatments to patients with complex nursing problems.
6. Participates in daily DMS huddles and unit specific staff meetings.
7. Participates in coaching and mentoring of colleagues or new employees.
8. Performs and documents within the electronic medical record.
9. Completes CPRS documentation and closes encounters by facility policy.
10. Provides timely patient follow-up.
11. Maintains confidentiality of patient information.
12. Seeking opportunities for self-improvement and participating in performance improvements.
13. Collaborates and communicates effectively with the Interdisciplinary Care Team.
14. Documentation is factual, objective, accurate, consistent, timely, and complete.
15. Documents and signs all patient related activity within tour of duty.
16. Assists with coordinating care of patients.
17. Adhere to ethical principles and standards in delivering care.
18. Communicate with team members regarding patient care.
19. Seeking opportunities for self-improvement.
20. Demonstrate mandatory and clinical competencies of the position.
21. Implements nursing interventions/orders in a timely and complete manner.
22.Recognizes changes in patient's condition and deviations from baseline that require immediate intervention.
23.Provide patient education related to managing risk factors, medications, procedures, and other medical regimens related to disease process/specialty clinic, within scope of practice.
24.Obtains Nail Certification within 12 months of hire.
25.Recognizes changes in patient's condition and deviations from baseline that require immediate intervention.
26. Apply infection control measures appropriately in delivering care.
27.Alerts providers (physicians/nurse practitioners) of abnormal signs and symptoms and/or abnormal test results to expedite care.
28.Collaborate with the Registered Nurse or Charge Nurse when the patient requires a higher level of care that is not manageable on the unit.
29. Gathers additional data (face to face, via phone, or video conferencing) related to patient needs for treatment determination of Surgical Specialty Clinic's needs.
30. Performs other duties, as assigned.
Preferred Experience:
1-2 yrs current Ambulatory Care Exp ( PACT or Surgical Specialty clinic: wound care, urology care, minor procedure, gynecology, etc.). Inpatient, SNIF or ED experience helpful.
Work Schedule: Monday - Friday, 0800 to 1630
