General Description:
Under the general supervision of the Quality Manager, the WOCN Program Coordinator is responsible for the primary coordination, planning, developing, implementing and evaluating comprehensive individualized Wound, Ostomy and Continence (WOC) Programs according to the Wound, Ostomy and Continence Nurses Society (WOCN) Position Statement: Role and Scope of Practice for Ostomy Care Providers, 2017. Applies knowledge from WOCN certification, current research, and evidence for clinical decisions and to achieve optimal outcomes for the wound care patient. Uses evidence-based benchmarks to evaluate, monitor, trend, and promote practice at departmental, organizational, and national levels. Incorporates theories and research in generating strategies to promote and improve healthcare practices for the wound care patient.
Key Responsibilities:- Provides expert hands-on care for individual patients with acute or chronic wounds. Conducts thorough assessments of patients' wounds and overall health in collaboration with nursing staff.
- In consultation with the physician, assists the primary nurse in developing an appropriate plan of care for comprehensive wound management based on patient needs.
- Serves as an expert clinician, consultant, and educator to improve patient care and outcomes.
- Provides leadership in the design of quality improvement initiatives to optimize outcomes of care for the wound/ostomy patient.
- Uses evidence-based knowledge and research findings to guide practice and develop strategies/interventions to manage care to achieve appropriate goals and outcomes for patients with acute or chronic wounds. Monitors and reports quality data/metrics.
- Works closely with the Multi-disciplinary team to achieve desired length of patient stay for related patient populations by advocating availability and access to specialty care services, resources, and supplies.
- Evaluates processes, policies, procedures, and protocols/guidelines for care of the patients; and recommends revisions when warranted.
- Develops and implements evidence-based educational programs for patients, staff, and other healthcare providers.
- Facilitates/coordinates use of systems and community resources to implement and enhance care of WOC patients across the continuum of care.
- Engages consumer alliance and advocacy groups in health teaching and health promotion activities for patients (e.g., ostomy support groups, diabetes support groups).
- Knows fire, disaster and safety procedures and regulations as pertains to the work area.
- Performs similar or related duties as assigned.
Requirements:Formal Education:- Bachelor Degree in Nursing Required
Work Experience:-
2 years to ‹3 years' experience in nursing
Required Licenses, Certifications, Registrations:- Valid State of FL RN License required.
- Registered Nurse Certification as CWON (Certified Wound, Ostomy Nurse) required upon hire or ability to obtain within 2 years of hire.
- Lean Six Sigma Green Belt preferred.
