Women's Program Case Manager

Qualifications

  • Extensive knowledge of social work practices, community resources, and relevant regulations
  • Strong leadership and supervisory skills
  • Excellent communication and interpersonal skills to collaborate with clients, staff, and community partners
  • Proficiency in Microsoft applications and database systems is required
  • Must complete a criminal background check

Job Responsibilities:

The case manager for women and children in our life recovery offers personalized support to families, often during crises such as homelessness, domestic violence, or involvement with the child welfare system. The primary goal is to assist the women and children in our program in identifying their needs, developing a service plan, and connecting with resources to ensure their safety, stability, and overall well-being.

Assessment and planning

  • Conduct comprehensive intake and needs assessments to identify a family's strengths and challenges.
  • Develop and implement an individualized case plan with clear, achievable goals in collaboration with the family.
  • Continuously monitor a client's progress, adjusting the service plan as circumstances change.
  • Coordinating the calendar for all appointments and effectively communicating with the treatment team.

Client support and advocacy

  • Provide crisis intervention and emotional support, using a trauma-informed and strengths-based approach.
  • Advocate for clients' rights and needs within the social services, healthcare, legal, and housing systems.
  • Assist with the navigation of legal processes related to child welfare, domestic violence, or family court proceedings.

Resource coordination and referrals

  • Assist families in applying for essential community resources, such as housing assistance, food programs (e.g., WIC, food stamps), and financial aid.
  • Coordinate services with schools, healthcare providers, legal advocates, mental health professionals, and other agencies involved with the family.
  • Arrange for and provide transportation for clients to appointments.

Case management and administration

  • Maintain accurate, up-to-date, and confidential case files and documentation.
  • Ensure timely completion of all necessary paperwork for the agency.
  • Record and track clients' progress to report on program effectiveness and outcomes.
  • Participate in regular staff meetings and training sessions to maintain up-to-date knowledge.

Essential qualifications and skills

  • Education and experience: A bachelor's degree in a related field such as social work, psychology, or human services is typically required. Experience working with at-risk populations, including families, women, and children, is also essential.
  • Empathy and compassion: The ability to listen and build trust is crucial for understanding and reacting appropriately to a client's needs.
  • Excellent communication skills: This includes active listening and the ability to clearly convey complex information to clients, their families, and other professionals.
  • Time management and organization: The role requires managing a large caseload of clients and prioritizing tasks to meet deadlines and administrative requirements.
  • Problem-solving: Case managers must be resourceful and think critically to overcome challenges and find creative solutions for each family's unique circumstances.
  • Cultural competence: The ability to work respectfully and effectively with a diverse range of clients from varying socioeconomic backgrounds is vital.

Job Type: Full-time

Pay: $13.50 - $15.50 per hour
