We are seeking a highly skilled and compassionate Wound Care Nurse Practitioner (NP) to deliver specialized wound care services in collaboration with facility wound nurses and interdisciplinary healthcare teams. The NP will manage acute and chronic wounds, including pressure injuries- all stages, rashes, skin tears, malignant wounds, diabetic foot ulcers, venous stasis ulcers, and post-surgical wounds, using evidence-based practices to optimize patient healing and outcomes.
Key Responsibilities:
1. Patient Assessment & Care Planning
- Conduct comprehensive health assessments and physical exams specific to wound care needs.
- Develop and evaluate individualized treatment plans tailored to wound severity, etiology, and patient health status.
- Order and interpret appropriate diagnostic tests to support wound evaluation and management.
- Prescribe medications, topical agents, and therapeutic interventions as needed.
2. Wound Management
- Perform advanced wound care procedures such as conservative sharp debridement, prescribing of appropriate wound treatments, and negative pressure wound therapy.
- Monitor wound healing progress with thorough documentation and photography.
- Adjust treatment protocols based on healing trajectory and patient response.
- Utilize evidence-based clinical guidelines to guide wound interventions.
3. Patient & Family Education
- Educate patients and caregivers on wound care best practices, hygiene, pressure offloading, and nutritional support.
- Promote preventive strategies to reduce wound recurrence.
- Offer emotional support and guidance throughout the treatment process.
4. Interdisciplinary Collaboration
- Coordinate care with physicians, nurses, physical therapists, dietitians, and case managers.
- Participate in multidisciplinary care rounds and case conferences as needed by the facilities.
- Facilitate referrals to specialty providers when indicated.
5. Quality Improvement & Professional Development
- Contribute to wound care quality initiatives and clinical audits.
- Stay informed on the latest advances in wound management through continuing education.
- Assist in developing and updating wound care policies and clinical pathways.
6. Documentation & Regulatory Compliance
- Maintain accurate and timely electronic health records in accordance with legal and institutional requirements.
- Ensure adherence to all state and federal guidelines, including CMS and Joint Commission standards.
Qualifications:
Education & Licensure:
- Master’s degree in nursing from an accredited Nurse Practitioner program.
- Current and unrestricted California Nurse Practitioner license.
- Active NPI number and DEA registration (or eligibility).
Certifications:
- National board certification as an NP (e.g., ANCC or AANP).
- Certification in wound care (e.g., WCC, CWCN, CWS) preferred but not required to begin.
Experience:
- Prior experience in wound care, dermatology, or surgical nursing is strongly preferred.
- Familiarity with electronic health record systems and wound documentation tools.

Pay: $40,000.00 - $160,000.00 per year
