Wraparound Facilitator

Title: Wraparound Facilitator
Location: Anywhere in Colorado
Structure: 90% Remote | 10% In-Person

About the Role:

Wraparound facilitation is rooted in a holistic, strengths-based approach to supporting youth and families through individualized and intensive care coordination. As a Wraparound Facilitator, you will meet families where they are—often during challenging or crisis-driven moments—learn about their goals, needs, and strengths, and support them in turning their vision for their child into action.

A core part of this work involves convening and facilitating consistent team meetings that bring together families, natural supports, and professionals. These meetings foster shared ownership, collaboration, and accountability in how each team member contributes to the family’s mission.

Outside of team meetings, you will work closely with families to navigate complex systems, develop creative solutions, connect with service providers, and offer steady, supportive guidance. Our goal is to use wraparound as a meaningful modality to improve outcomes for youth and families across Colorado and to promote equity for vulnerable populations.

Through this work, we partner with the state of Colorado in advancing long-term solutions that help children remain healthy, safe, and supported in their homes and communities rather than entering out-of-home placements.

Core Responsibilities (not limited to):

  • Facilitate team-based wraparound meetings
  • Conduct strengths- and needs-based assessments
  • Develop, implement, and adapt individualized care plans
  • Maintain consistent communication with families and team members
  • Support youth and families in meetings outside of formal wraparound sessions
  • Track progress and document updates to care plans
  • Build in-person rapport with youth and families
  • Develop and update crisis prevention plans
  • Uphold a family-defined vision of success
  • Align practice with high-fidelity wraparound principles
  • Engage in collaborative consultation, training opportunities, and professional development

Basic Qualifications:

  • Bachelor’s degree (or higher) in social work, human development, psychology, or a related field
  • Prior experience in wraparound, case management, care coordination, or related youth & family services
  • Ability to work independently and manage a flexible caseload
  • Strong communication, facilitation, and organizational skills

Start Date:

  • Mid April
  • Early May
