Youth Community Support Provider:
FT position for Youth Community Support (case management) with comprehensive insurance benefits, eligibility for generous paid leave, a retirement plan with employer match, EAP, and productivity bonus.
Overview:
Direct services to clients and families, including, but not limited to, needs assessment treatment planning, skill development, service monitoring, referral and linkage, interagency networking, advocacy and crisis prevention. Requires work in clients' homes within the community and in schools. Staff must be able to deliver services in person, by phone, and via on agency-approved telehealth platform, based on client or family preference. Responsible for meeting established productivity standards, including billable service units, in alignment with agency financial goals. Service planning to include case coordination and recording documentation of assessments, and progress for client in achieving treatment goals. Team/staff meetings, receiving supervision, travel and training. Perform computer data entry. Timely and accurate completion of time and leave reporting/forms. Regular attendance and adherence to work schedule. Ability to work a flexible schedule, with some evening hours required to accommodate family schedule.
Requirements:
Minimum:
Bachelor's degree in a mental health-related field such as social work, counseling, psychology, or human/family services is required, no licensure, or prior experience necessary. A high school diploma with a minimum of three (3) years of relevant experience will also be considered. Degrees in related fields such as education, criminal justice, or connections may be considered with applicable experience. Strong written and verbal communication, assessment, planning, organizational skills and teamwork are essential. Proficiency in Microsoft Office Word is preferred; and ability to perform computer data entry is required. Criminal background, drug screening and BMV checks and clearances required. A valid driver's license, excellent driving record, and proof of meeting state minimum auto insurance liability coverage limits are required. E.O.E.
Preferred:
Bachelor's degree in a mental health related field such as social work, counseling, psychology, human/family services, with one year of paid experience, preferably with the service population, and LSW.
