Youth Program Case Manager

Job Summary:

Through a strong Christian worldview, assists residents in addressing spiritual, physical, behavioral, and emotional needs. Provides dedicated care to residents; identifying needs, determining eligibility and referral for a variety of social services, and guiding youth through the program helping to provide holistic stability. Supports relevant programming for all youth, including gospel-centered lessons, bible-based character development, and academic improvement.

Supervisory Responsibilities:

  • This role does not currently supervise other staff; however, this role will supervise volunteers and interns as applicable.
  • Coaches, corrects, rewards, recognizes, and reviews volunteers and interns in accordance with TCM’s Core Values & policies.

Essential Job Functions:

Determines Eligibility and Develops Service Plans

  • Performs assessments that measure development of Pathways youth.
  • Develops resident Individual Service Plans after a needs assessment interview.
  • Interviews residents to determine their child’s needs and refers them to appropriate community resources.

Assists in Coordinating Care

  • Acts as liaison between residents, organizations, and agencies.
  • Provides emergency assistance, crisis intervention, and needed referrals.
  • Collaborates with Laura’s Home team to guide and support residents.
  • Updates resident files by recording each contact made with residents and families, and maintains records required for follow-up.
  • Provides a timely response to the needs of all residents on case load.
  • Serves as an advocate for individuals in the community, working with other community organizations to address issues facing assigned residents.

Supports Programming

  • Assists Pathways programming by supporting, serving, and ministering to our youth and families in a Christian environment conducive to transformation.
  • Attends daily pathways programming to build quality, caring relationships with the youth and families in order to enhance overall effectiveness of Pathways programming.
  • Assists in the infant and toddler, and preschool rooms as the census dictates.
  • Understands and enforces policies/procedures to maintain discipline and ensure safety at all times.

Other Responsibilities

Actively grows and pursues continued education in community-based services and domain knowledge.


Education/Experience:

  • Completed or working towards a bachelor’s degree in Social Work or related field is required.
  • Licensed Social Worker is preferred.
  • 2 years experience in youth work required.
  • 3 years of service in the Social Work field preferred.
  • Knowledge in trauma-informed care and crisis intervention required.

Additional Eligibility Qualifications:

  • Meets requirements to be an approved driver of TCM vehicles is preferred .
  • Limited time off over summer and school breaks.
  • Flexible schedule required in order to be onsite for programming hours, available to drive/chaperone at special events, field trips, available during programming hours, evenings and weekends.

Essential Physical and Mental Functions:

  • Remains in a stationary position for approximately 30% of the time
  • Stand or walk for up 70% of the time on various surfaces (tile, concrete, carpet)
  • Climb stairs.
  • Constantly bend/twist at waist/knees/neck to perform various duties.
  • Lift, carry, or move 50 pounds.
  • Constant use of both hands, fingers, and arms in reaching/handling/grasping.
  • Operates a computer as needed
  • Constant use of eyes (correctable vision to normal level required) to observe, read, interact with public and co-workers, view security monitors; includes hand/eye coordination.
  • Constant use of hearing (correctable hearing to normal level required) for day-to-day job functions
  • Read, understand, and clearly speak English; constantly use speech and hearing (correctable to normal level required) in communicating with public/co-workers, giving and receiving instructions, using phones.
  • Constant mental alertness and attention to detail required while setting priorities and following up on assignments.
  • Must possess effective written and oral communication and interpersonal skills with ability to deal with all levels of personnel and the general public in a professional and effective manner; must be able to use initiative and independent judgment within established guidelines.
  • Must be able to handle pressure of constantly working with individuals in crisis.
  • Must be able to handle pressure of interruptions in daily activities.
  • Subject to excessive noise

Spiritual Requirements:

  • Professes Jesus as Lord and Savior
  • Believes the Bible to be the inspired, infallible, & ultimate authoritative Word of God (II Timothy 3:16; II Peter 1:21)
  • Patterns life after the example of Jesus and the commands and principles found in the Bible – consistent with TCM’s doctrinal statement, statement of faith, and lifestyle principles
  • Desires to serve the Lord
