Salary: £14.00–£14.50 per hour (depending on experience)
Hours: Full Time – 37.5 hours per week, Monday to Friday
Work Location: Office-based with travel to service users' homes and the community as required

As our service continues to grow, we are looking for an organised, compassionate and proactive Care Coordinator to join our friendly and supportive team.

This is a 6-month fixed-term contract with a genuine opportunity to become a permanent member of the team, subject to business needs, performance and the successful completion of the fixed-term contract.

About the Role

This is a varied and rewarding role combining care coordination, care planning, quality assurance and office administration.

You will play a key role in ensuring our clients receive safe, effective and person-centred care while supporting the smooth day-to-day operation of the service.

Working closely with service users, families, care staff and healthcare professionals, you will help ensure that high standards of care are maintained and that our service continues to meet the expectations of our clients and the Care Quality Commission (CQC).

No two days are the same, making this an ideal opportunity for someone who enjoys variety, problem-solving and working collaboratively.

Key Responsibilities

  • Coordinate the day-to-day delivery of domiciliary care services.
  • Produce, review and maintain accurate, person-centred care plans and risk assessments.
  • Complete client assessments, reviews and welfare visits.
  • Liaise professionally with service users, relatives, healthcare professionals, local authorities and other external agencies.
  • Support, coach and guide care staff to deliver outstanding care.
  • Undertake spot checks, competency observations and quality assurance visits.
  • Maintain accurate client and staff records using Birdie and other electronic systems.
  • Support rota coordination and assist with emergency cover arrangements where required.
  • Respond appropriately to incidents, complaints, safeguarding concerns and changes in client needs.
  • Support recruitment, onboarding and general office administration.
  • Monitor compliance with Company policies, procedures and CQC Fundamental Standards.
  • Provide direct care in exceptional circumstances where operationally required.
  • Work closely with the Registered Manager to continuously improve the quality of our service.

Essential Requirements

  • Excellent written and verbal communication skills.
  • Strong organisational and time management skills.
  • Excellent attention to detail.
  • Good IT skills, including Microsoft Office.
  • Ability to produce accurate documentation and maintain confidential records.
  • Ability to build positive working relationships with service users, families, colleagues and professionals.
  • Full UK driving licence and access to a vehicle.
  • Willingness to undergo an Enhanced DBS check.

Desirable

  • Previous experience within domiciliary care, health or social care.
  • Experience producing care plans and risk assessments.
  • Experience using Birdie or another electronic care management system.
  • Experience in care coordination, scheduling or office administration.
  • Level 3 Diploma in Adult Care (or equivalent).
