The Autism Case Manager will help adults with autism and other disabilities build the skills they need to live more independent, connected lives. This role will provide case management, coaching, and supportive counseling to adults, using a clinically informed approach.

This role will support clients in critical areas of daily living, including independent living and social skills, while coordinating services with JF&CS programs and community agencies to ensure each client receives the right resources and support.

The Autism Case Manager position is a meaningful opportunity to help empower neurodiverse individuals to achieve greater independence and reach their personal goals.

This is a full-time Monday-Friday position with no on-call duties. The hours are flexible; with some early evening hours required 1-2 days/week. Clients will be seen in person throughout the Greater Boston area, out of our Waltham office, and virtually.


Highlights

  • $58,000-$61,500/year salary, paid at an hourly rate
  • Full Time, 40 hour per week position working in the community and out of our Waltham location w/ milage reimbursement

Principal Duties and Responsibilities

  • Provide skills training in the areas of financial management, social skills, organization strategies, and advocacy skills. This includes using multimodal methods of communicating and adapting communication to address specific client needs.
  • Responsible for writing treatment plans, completing daily documentation, and regular review of individual service plans.
  • Perform case management tasks including coordination and assistance with application for health care, housing, and SNAP (food stamp) benefits; as well as coordinating these services when several services/providers are involved.
  • Establish a trusting and supportive relationship with clients from diverse backgrounds; provide high quality, compassionate, and ethical care.
  • Collaborate with families, DDS, other service providers, and other collateral contacts..
  • Meet with clients in the community, their homes, Waltham office, or virtually.
  • Participate in trainings and team meetings, and be an active, collaborative member of the JF&CS case management teams.

Requirements:

  • Higher education or relevant work experience in Social Work, or another related field (mental health, education, psychology, human services). Master’s degree required.
  • Experience working with individuals with Autism Spectrum Disorder, other intellectual/developmental disabilities and/or psychiatric conditions.
  • A valid driver’s license and a reliable, insured vehicle (mileage is reimbursed).
  • Proficiency with Microsoft Office Suite, especially Word and Outlook.
  • Strong verbal and written communication skills; organizational skills; and high level of attention to detail.
  • Must be able to work independently and as a part of a group.
  • Capability with electronic case record software for agency documentation (Salesforce); training will be provided
