We are looking for a motivated and compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team, based at Paulton Memorial Hospital. This is a rewarding opportunity to make a real difference to patients and their families by supporting safe, timely discharges into community services across our BSW Community Hospitals.
In this role, you will work closely with nurses, therapists, social workers and other members of the multidisciplinary team to coordinate patient flow and ensure everyone has the right information at the right time. You will play a key part in keeping discharge processes running smoothly, maintaining accurate records, completing referrals and supporting clear communication across services.
You’ll also build positive relationships with patients and their families on the wards, helping them understand the discharge process and keeping them informed throughout their stay.

Paulton Memorial Hospital is located in the heart of the village of Paulton, set in the beautiful Somerset countryside close to the Mendip Hills. The area offers a great balance of rural charm and convenience, with good transport links to Bath, Bristol and the surrounding towns, as well as plenty of local amenities, green spaces and walking routes.
