We are looking for a motivated and compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team, based on Longleat Ward at Warminster Community Hospital. This is a rewarding opportunity to make a real difference to patients and their families by supporting safe, timely discharges into community services across our BSW Community Hospitals.
In this role, you will work closely with nurses, therapists, social workers and other members of the multidisciplinary team to coordinate patient flow and ensure everyone has the right information at the right time. You will play a key role in supporting smooth discharge processes, maintaining accurate records, completing referrals and ensuring clear communication across services.
You will also build strong, supportive relationships with patients and their families on the ward, helping them to understand the discharge process and keeping them informed throughout their hospital stay.
About the location
Warminster Community Hospital is home to Longleat Ward and is located in the historic market town of Warminster, on the edge of Salisbury Plain. The area offers excellent access to beautiful countryside, including nearby Longleat Estate, while also benefiting from good transport links to Bath, Salisbury and Westbury. Warminster town centre provides a range of local shops, cafés and amenities, making it a pleasant and convenient place to work.
