Business Analyst – Value Based Care (Health Cloud) Position Overview

We are seeking a highly skilled Business Analyst (BA) to join our Scrum team focused on implementing Value Based Care (VBC) solutions on Salesforce Health Cloud.

The ideal candidate will bring strong healthcare domain expertise, particularly in clinical workflows and EHR/EMR data models, and act as the bridge between business stakeholders, EHR teams, and the Salesforce development team.

This role requires a deep understanding of patient-centric data (patient demographics, encounters, lab results, medications, and appointments) and how that data fuels Care Plans, Care Goals, and Actions in a Value Based Care context. Key Responsibilities

• Serve as the liaison between clinical/EHR stakeholders and the Salesforce Health Cloud development team. • Collaborate with EHR/EMR teams to extract patient-related data (demographics, lab results, medications, encounters, appointments, etc.) and map it into Health Cloud.

• Define and refine requirements for Value Based Care workflows, including Care Plans, Care Goals, Care Gaps, and Care Actions.

• Work with Product Owner to groom and prioritize backlog items aligned to VBC program objectives.

• Facilitate requirement workshops, story refinement sessions, and sprint planning with cross-functional teams.

• Translate clinical and regulatory requirements into user stories, acceptance criteria, and process flows for Health Cloud implementation.

• Support QA and UAT by validating test cases and ensuring delivered functionality aligns with business expectations.

• Act as the subject matter expert (SME) for Value Based Care concepts (risk stratification, quality measures, care coordination, patient engagement).

• Provide thought leadership on leveraging Health Cloud capabilities for population health and VBC enablement. Required Skills & Experience

• 5+ years of Business Analyst experience in Healthcare IT, preferably Care Management workflows.

• Strong knowledge of clinical workflows and EHR/EMR systems (Epic, Cerner, Allscripts, MEDITECH, etc.).

• Hands-on experience with clinical data models: patient demographics, encounters, labs, medications, and appointments.

• Understanding of Value Based Care programs (e.g., ACOs, CMS Quality Measures, Risk Adjustment, Care Coordination).

• Familiarity with Salesforce Health Cloud data model, including Care Plan, Care Goal, Care Gap, and Care Action objects

. • Proven ability to write clear and concise user stories, process flows, and data mapping documents.

• Strong communication, stakeholder management, and facilitation skills. • Experience working in Agile/Scrum delivery models.

Preferred Skills

• Salesforce certifications (Admin, Health Cloud Consultant, BA, or related). • Experience with FHIR/HL7 integration patterns.

• Knowledge of Healthcare regulatory requirements (HIPAA, CMS, NCQA, HEDIS).

• Prior experience in population health management platforms
