Care Coordination Case Manager

Job Summary
Provides comprehensive case management, service coordination in assessing the needs of clients, and linkage to appropriate services.

Responsibilities

1. Participates in departmental projects including Community Care Program (CCP), Managed Care Organizations (MCO), Caregiver Programs and other special projects.

2. Assess client needs including physical, social, health and enviornmental needs and how these interact by completing in-home assessments.

3. Targets services to those aged 60 and over, those who are isolated, in poor health, have income at poverty level, and those belonging to minority groups.

4. Connects individuals to available services and programs in their community which they may qualify.

5. Turn in folders weekly and meet the monthly folder quota.

6. Encourages and assists persons to gain access to needed services, including:

a) Making referrals and providing on-going follow ups to ensure services are in place.

b) Assist in completing all necessary forms for state and federal benefit programs

c) Advocate on behalf of clint to gain access to benefit programs.

7. Maintains case notes and develops a person -centered care plan for each client.

8. Advocates for clients in problem situations, including representing clients in appeals, and reconsiderations, and makes all attempts to meet client's needs including through alternate resources.

9. Accepts case assignments as given with consideration to case load, mileage and time guidelines.

10. Attends necessary training as provided by the Illinois Department on Aging.

11. Adheres to federal and state guidelines, policies, and deadlines.

12. Participates in case conferences, case planning and implementation, coordination of resources, ensuring alternate services and monitoring client's progress.

13. Assess clients for eligibility for access to additional programs such as home delivered meals, money management, senior companion, etc.

14. Completes daily confidential note recording for clients.

15. Participate in quality reviews.

16. Coordinates with other agencies as needed.

17. Cultivates community liaison relationships.

18. Serves as resource person for persons aged 60 and over and their families in the community.

19. Keeps statistical client data necessary for appropriate records and reports to manager.

20. Follows agency policies and procedures.

21. Projects good image for agency.

22. Maintains credibility and integrity of organization.

23. Performs all other duties as assigned.

Pay: From $48,500.00 per year
