Care Coordinator Nurse (Home Healthcare) Lahore

Job Overview: Considracare Pakistan is seeking a dedicated and experienced Care Coordinator Nurse to manage client services and caregiver operations. This is a management-level role requiring strong coordination, clinical understanding, and leadership skills in a home healthcare or hospital setting.

Key Responsibilities:

• Manage daily caregiver rosters, scheduling, and shift planning
• Coordinate with clients and caregivers for smooth service delivery
• Conduct patient assessment visits when required
• Handle incoming calls from clients and caregivers professionally
• Ensure timely resolution of caregiver attendance, arrival, and checkout issues
• Maintain accurate client and caregiver records on the Considracare platform
• Monitor service quality and ensure client satisfaction

Working Hours:

• 9 hours daily (On-site)
• Sunday off
• Must be available on-call (24/7) for urgent situations such as emergency visits or caregiver issues

Eligibility Criteria:

• BSN (BSc Nursing) or General Nursing Diploma
• 2–4 years of relevant experience
• Prior experience in a management/coordination role in a home healthcare company or as an in-charge nurse in a hospital
• Strong communication, organizational, and problem-solving skills

Preferred Skills:

• Experience in home healthcare coordination
• Familiarity with digital systems/CRMs
• Ability to handle pressure and manage multiple cases simultaneously
