About the Role

CareBound Home Health is seeking a highly organized and proactive Care Coordinator / Operations Coordinator to support and manage daily operations.

This role is designed to take ownership of scheduling, coordination of care, administrative tasks, and communication—allowing leadership to focus on growth and partnerships.

You will play a critical role in ensuring that clients receive consistent, high-quality care while keeping internal operations running smoothly.

Key ResponsibilitiesScheduling & Staffing

• Create and manage caregiver schedules for all active clients
• Handle call-offs, no-shows, and last-minute coverage
• Match caregivers to clients based on needs and availability
• Ensure all shifts are filled and properly documented

Care Coordination

• Communicate with clients and families regarding services and schedules
• Support implementation of care plans
• Monitor service consistency and report issues
• Coordinate with caregivers to ensure proper care delivery

Referrals & Intake Support

• Respond to new client referrals in a timely and professional manner
• Collect and organize intake information
• Assist with onboarding new clients into the system
• Maintain communication with referral sources as directed

Documentation & Administrative Support

• Track and follow up on caregiver documentation (care notes, visit notes, etc.)
• Ensure records are complete, accurate, and up to date
• Assist with organizing Plan of Care and service documentation
• Maintain client and caregiver files

Caregiver Support & Hiring Coordination

• Assist with caregiver onboarding and orientation coordination
• Maintain caregiver availability and contact records
• Serve as a point of contact for caregiver questions and support

• Experience in home care, healthcare, or scheduling is preferred

Work Environment

• Hybrid role (combination of in-office and remote work)
• Must be available and responsive during scheduled work hours
• Fast-paced environment with real-time problem solving

• All client shifts are consistently covered
• Caregiver documentation is completed and up to date
• New referrals are responded to quickly and professionally
• Communication between clients, caregivers, and the office is smooth
• Daily operations run without constant oversight
