Role Summary
This role offers a meaningful opportunity to make a direct and positive impact on the lives of older adults and individuals with complex care needs. Caregivers support patients by providing essential daily care services, promoting comfort, safety, and dignity. You will work as a valued member of a compassionate and collaborative team while making a difference every day.
Duties
Provide personal care and escort assistance based on each patient’s individualized care plan, including grooming, dressing, bathing, toileting, peri-care, housekeeping, meal preparation/feeding, laundry, and related tasks.
Observe and report changes in a patient’s condition to the team to support coordinated and effective care.
Demonstrate safe practices by using proper body mechanics, following universal precautions, and maintaining a safe environment.
Interact with patients, families, and the team in a positive, professional, and supportive manner; encourage engagement in conversations and activities.
Perform additional related duties as assigned.
Required Qualifications
Minimum 1 year of experience working with the elderly or frail population.
Ability to work both independently and collaboratively as part of a team.
Flexibility to work in different care environments, including in-home or center-based settings.
A positive, compassionate, and conscientious attitude with a strong commitment to patient-centered care.
Basic conversational English skills; testing may be required.
Bilingual skills may be required depending on patient needs; language testing may be required.
Ability to translate or interpret basic information for supervisors, patients, and fellow caregivers (testing may be required).
Ability to document care-related information in basic English using designated forms or electronic health records.

Desired Qualifications
Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification
Familiarity with coordinated care models serving elderly populations
Bilingual
Driver license
Functional Requirements & Working Conditions
Ability to stand, reach, walk, push, pull, lift, grasp, talk, hear, and perform repeated wrist/hand motions.
Must have sufficient visual acuity.
Must be able to safely lift or move weights within personal physical ability, using lifting devices or requesting assistance when needed.

Location & Work Type
On-site role providing services in the day center settings. May require flexibility to work across multiple care locations.
Schedule Details
Full Time Monday through Friday 8 AM - 4:30 PM
