Job Summary and Qualifications

POSITION SUMMARY:

Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psychosocial approach through the continuum of care. Performs concurrent clinical review todetermineappropriateness of admission and continued need for care delivery in the acute care setting. Performs telephonic clinical reviews with claims insurance personnel tofacilitatereimbursement for the delivery of patient care services. Assesses thepatient for discharge planning needs and referrals to the interdisciplinary team. Supports collaboration and communication among the patient/family and entire health care teamregardingthe patient’s plan of care, continued needs post-discharge, andfacilitatesthose services required. Discharge planning will begin at the timeof (or prior to)admission, and reassessed ongoing throughout the course of hospitalization. Identifiesquality and/or risk issues and reports these to the appropriate individual/department.
