The role of the Case Manager is to assist consumers committed under Wisconsin Statute Chapter 980 to independent community living. Case management staff will coordinate services and treatment in collaboration with the Department of Community Corrections, sex offender treatment providers, supportive care agencies, and service providers. The Case Manager is responsible for pre-release planning, case planning, and helping the consumer develop skills for community living. making individualized referrals to services that address the needs of their consumer. Clients may have developmental disabilities, experience mental health illness, and/or substance use disorders and require a Case Manager who is both empathetic and compassionate. The Case Manager should have knowledge of resources in Southeastern Wisconsin and act as an advocate in pursuing resources for the consumer.

Responsibilities:

  • Complete introduction of Motivational Interviewing (MI) Fidelity policy as part of onboarding
  • Complete MI training, participate actively in individualized MI learn plan with MI Coach, use MI in daily interactions, and record quarterly MI practice audio sample for review and feedback
  • Complete and utilize results of assessments to develop individualized case plan with consumer
  • Meet with consumers weekly utilizing a combination of face-to-face, video conference, and/or phone contacts
  • Manage a caseload of up to 7 consumers
  • Learn and seek to master use of Motivational Interviewing with consumers
  • Maintain a working knowledge of service providers and make referrals to appropriate providers, based on client’s needs
  • Complete accurate written case notes and staffing reports
  • Coordinate and collaborate with multiple stakeholders and service providers
  • Monitor services and ensure consumer needs are being met by providers utilizing in person and electronic communication
  • Review and approve client budgets and financial requests
  • Attend meetings with Department of Health Services on a monthly basis
  • Other duties as assigned

Qualifications, Competencies, and Requirements:

  • Bachelor’s degree in Criminal Justice, Social Work, Psychology, or related Human Services field
  • Applied knowledge and understanding of mental illness, substance abuse, and the effects of poverty on various socioeconomic groups
  • Working knowledge of community resources relevant to the needs of the client which include sex offender treatment, mental health treatment, housing, vocational, employment, and health services
  • Working knowledge of Microsoft Office (including Outlook, Word, and Excel)
  • Ability to communicate professionally & accurately, both orally and written
  • Organized, attentive to detail, and ability to work independently
  • Previous experience working with criminal justice-involved clients preferred
  • Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities
  • Pass a pre-employment Wisconsin Caregiver Background check
  • Valid driver’s license and automobile with insurance
  • Work from home with the ability to travel up to 40% of workweek

The Case Manager will need to be able to attend trainings and meetings at the Administration Office in downtown Milwaukee, and occasionally in other parts of the State. This position will include on-call rotation. Office equipment will be provided for remote work and requires a private space to use as a home office due to confidentiality.

Supervised release works with clients who have convictions of a sexual nature, and we meet with clients in their homes, over the phone, and at locations in the community. Case managers in Supervised release participate in DHS’ Motivational Interviewing (MI) implementation project, including ongoing coaching, and recording of MI Practice samples. Case managers can be credentialed in Basic and Advanced MI Proficiency.
