Job Responsibilities: 
<ul>
<li>Identify, plan, coordinate community based services for members</li>
<li>Responsible to contact members and informal care givers</li>
<li>Conduct in home visits and assessments</li>
<li>Maintain member records and resolve variances</li>
<li>Meet with members’ Primary Care Physicians quarterly</li>
</ul>
Qualifications: 
<ul>
<li>BS in social work or related human service field is required OR</li>
<li>Valid Georgia LPN License</li>
<li>Experience in social work, home and community based services, healthcare or geriatrics preferred.</li>
<li>Valid Driver's License</li>
<li>Reliable Transportation</li>
</ul>
