Compensation Range: $24/hr - $26/hr commensurate with experience

Overview:
The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to members with substance use and other chronic health conditions. Other interventions are Healthy Conversations, AOD Counseling and Seeking Safety education as eligibility to deliver them allows. The Case Manager will act as an intermediary between providers and members – serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, and more.
This role will work with Ryan White and Harm Reduction prevention services for high-risk of HIV populations. This position is scheduled for 9AM to 5PM from Monday to Friday
