We're growing fast and looking for a motivated, organized, and people-focused Social Worker / Case Manager who's ready to make a meaningful difference in the lives of elderly residents and their families.

Why You'll Love This Role

You won't be stuck behind a desk all day. You'll be out in facilities, building relationships, solving problems, and making sure every resident's care journey is smooth and supported.

You'll play a critical role in:

• Coordinating all aspects of resident care and managing projects and programs offered to residents including but not limited to chronic care, and GUIDE
• Guiding families through paperwork and insurance processes
• Collaborating closely with clinical teams and intake coordinators
• Arranging DME and ensuring non-clinical needs are handled efficiently
• Being the steady, organized, compassionate presence families rely on

What We're Looking For

• Someone who genuinely enjoys working with the elderly population
• Highly organized and proactive — you stay three steps ahead
• Comfortable making phone calls and communicating with families and facilities
• Tech-comfortable (EMR experience a plus)
• Willing to travel locally to facilities

Bonus Points For:

• Experience in case management or healthcare
• BSW (preferred but not required)
