Job Title: Wellness Coach / Case Manager

Location: Brooklyn, NY

Pay Rate: $22.00 – $24.00/hour (Contract)

Schedule: Sunday - Tuesday and Thursday (10am-6pm) and Wednesday 2pm-10pm. (Fridays and Saturdays Off)

About the Role

Pride Health is seeking a Wellness Coach / Case Manager to support individuals in the Bronx, NY. This role focuses on helping consumers access vital community services such as medical, psychological, vocational, housing, and daily living support. You will provide counseling, advocacy, and follow-up to ensure consumers can live meaningful, independent lives.

Key Responsibilities

• Manage caseloads and create individualized support plans
• Provide supportive counseling, problem-solving, and crisis intervention
• Accompany consumers to appointments and assist with daily living needs
• Advocate for consumers while promoting self-advocacy
• Coordinate community-based services and activities for social and skill development
• Document services and collaborate with therapeutic support teams
• Assist with moves and provide on-call crisis support when needed

Qualifications

Required:

• Minimum 2 years’ experience in behavioral health
• Proficiency in Microsoft Office and general computer applications

Preferred:

• Experience with mental illness, substance use, or homelessness
• Background in case management, community outreach, or supportive housing
• Bilingual (English/Spanish) strongly preferred
