Aging Solutions, Inc. Office of the public guardian - is currently looking for individuals seeking career opportunities in the field of guardianship. We are looking for Case Managers who are passionate about providing outstanding case management duties to protected persons/vulnerable adults who have been judicially determined to lack the capacity to make decisions for themselves and lack the ability to manage their own care and affairs. The case manager is tasked with duties of some essential health and safety requirements of the "Protected persons." Our county service areas cover the span of – Brevard, Hillsborough, Manatee, Pasco, Pinellas, and Sarasota. Most cases are in Hillsborough and Pinellas counties.

Job responsibilities:

Responsible for managing daily care and needs of protected persons

Conduct monthly visits with protected persons and their caregivers at their respective homes or residential facilities

Connect protected persons with professional services and maintaining consistent communication with caregivers to assess their wellbeing and needs.

Attend care plan meetings with multi-disciplinary staff at respective facilities

Effectively maintain and manage assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the protected persons served.

Requirements:

· Bachelor’s degree in Social/behavioral science or equivalent area of study

· Three to Five years of experience in case management

· Friendly and upbeat personality

· Ability to multitask

· Travel is required and applicants must have reliable car for field visits (Mileage is reimbursed for travels)
