Job Title:Case Manager
Work Site:Johnson County Office (JCO)
Area of Focus:Behavioral Health
Program(s):Supportive Housing program (820/205) provides adults living
with moderate levels of mental illness or disability with 24/7 services
with the goal of helping them transition to self-sufficiency or to a
less restrictive residential setting.
Immediate Supervisor:Community Transitions Director - Daniel Hyson
Hours of Work:Full-time, 40 hours/week. Peer crisis rotation.
Minimum Qualifications:High School Diploma or GED. Qualifies as a
Rehabilitative Services Associate (RSA).
Experience & Credential Requirements:Qualifies as a Mental Health
Professional (MHP) within 18 months of hire. Previous case management
Experience Is Preferred.
Skills & Knowledge:Must have knowledge of case management processes,
area resources/service systems, word processing and basic computer
skills. Employee must be able to assist the QMHP in addressing
individual client treatment needs and document clients progress toward
the goals and objectives identified on the Individual Treatment Plan.
Excellent written and oral communications, ability to teach basic life
Skills And Problem Solving Strategies Are Required.
Special Requirements:Must be 21 years of age, maintains a valid drivers
license and vehicle liability insurance, maintain First Aid & CPR
certification, must complete required registry clearances and criminal
fingerprint background checks, travels to all agency sites when
appropriate, schedules evening or weekend hours as necessary.
Jobduties Include, But Are Not Limited To
Provides outpatient mental health case management and community
support services throughout the clients stages of treatment with a
focus on engagement, care coordination, and discharge planning.
Intervenes to change attitudes, beliefs and behaviors of clients to
improve individual and family level of functioning.
Facilitatestransportation services to assist clients in connecting
withinternaland external resources.
Effectively teaches life-management and daily living skills to
clients that translate to their natural environment with an emphasis
on developing natural supports.
Ensures that regular updates as it relates to assigned clients
engagement is provided to program coordinator without prompting.
Maintains networking and referral contacts with other human service
providers bothinternally and externally while assessing barriers for
successful engagement, treatment, and discharge.
Completes required monthly direct service hours to clients.
Completes all required forms and documentation in a timely and
efficient manner.
Facilitates recreation/socialization and cultural activities such as
providing enjoyable outings that prepare and encourage clients to
engage in activities independent of the agency.
Facilitates linkage and referral to appropriate resources and
opportunities as identified in the clients treatment plan.
Demonstrates active involvement in team/program meetings ,
supervision, and agency/community events and activities as assigned.
Will have the ability to screen, assessand provide interventions for
clients that are exhibiting symptoms of trauma.
Reports all incidents of suspected abuse or neglect.
