Case Manager

Overview
The Prevention Case Manager works directly with students in multiple Ulster County schools to support healthy decision-making and prevent youth substance use. This role combines classroom-based instruction in social and emotional learning with individual early intervention sessions for students who are at risk.

Prevention Case Managers use evidence-based prevention strategies to help young people build skills and enhance internal motivation to make positive changes. Comprehensive training and supervision are provided, as are opportunities for professional growth and advancement.

This position is ideal for someone who enjoys working with young people, building relationships in school communities, and making a positive impact in young people’s lives.

Responsibilities

• Deliver evidence-based social and emotional learning curricula in school classrooms
• Conduct evidence-based early intervention assessment and counseling with students who may be at risk for a problem with substance use
• Complete training and obtain certification in designated evidence-based prevention programs; training and certification are provided and/or covered by the agency
• Connect students and families with appropriate support services and community resources as needed
• Advocate for students to ensure access to needed services
• Maintain accurate statistical records and documentation in compliance with regulatory requirements
• Attend case conferences, planning meetings and team meetings as requested by program leadership
• Participate in community partnerships, outreach activities and fundraising initiatives

Required Qualifications

• Bachelor’s degree and/or three years of experience working with young people
• Experience in the human services field, particularly with youth and families
• Ability to connect with and engage young people from diverse backgrounds

Preferred Qualifications

• Experience facilitating groups or classroom instruction for youth

Pay: $18.95 per hour
