At The Center for Progress and Excellence, we offer targeted mental health and case management services to at-risk individuals. Through our integrated approach to crisis intervention and outpatient services, our trained staff and licensed counselors can develop effective service and treatment plans that are unique for each individual to help them overcome challenges they may face in their daily lives.

SUMMARY The Case Manager will be a part of the multidisciplinary Mobile Crisis Response Team (MCRT) that includes master’s level clinicians, peer support specialists, and licensed clinicians. The Case Manager will provide direct client-centered support to individuals and families referred to the Mobile Crisis Response Team, to encourage the client’s involvement in after-care and provide a warm handoff to community services and agencies. The case manager will provide triage, crisis intervention, and linkage to services on demand, in any setting in which a crisis is occurring including homes and school settings.

ESSENTIAL DUTIES AND RESPONSIBILITIES PROGRAM DEVELOPMENT:

  • Engages individuals and families in the crisis and post-crisis planning process using evidenced-based practices
  • Conduct biopsychosocial intake assessments
  • Works with individuals, families, groups, and agencies to provide outreach, emotional support, service linkage, and referrals
  • Provides warm handoff to community resources, services, and supports
  • Triages phone calls and provides community resource information as well as supportive informal counseling
  • Facilitates stabilization and care coordination services post-crisis intervention
  • Coordinates with crisis counselors and team leaders so individuals receive mental health service appointments within 48 hours of intervention
  • Documents all client interaction within 24 hours of client-contact in the online EHR system
  • Assigns all notes in the EHR system to the respective clinician before the end of their shift
  • Appropriately utilizes all company forms, matrixes, and templates on a daily basis
  • Interacts with management to gather and provide information
  • Maintain a caseload of clients and conduct follow-ups within the necessary time frames
  • On occasion during specific shifts it may be required that the Case Manager conduct a SAFE-T assessment
  • Completes all the required trainings for this position within 60 days of hire and renews these trainings annually.
  • Participates in all staff meetings.
  • Reports client abuse, neglect, and exploitation as required by DCF.
  • Other duties as necessary, when they do not conflict with client services
  • Ability to drive and have reliable transportation

QUALIFICATIONS To perform this job successfully, an individual must have the ability to work effectively, independently, and in a team environment. Excellent verbal and written communication skills. Excellent interpersonal skills. Case Managers must be able to effectively connect with individuals and families to develop effective service plans and follow through with clients. Requires proven professionalism and the ability to deal effectively with clients, families, and a variety of internal and external resources. This position requires driving and the use of reliable transportation.

EDUCATION, EXPERIENCE & COMPETENCIES REQUIRED

  • Bachelor of Social Work/Psychology or mental health related field from an accredited institution required for case managers.
  • Valid Florida Driver’s License
  • Training and or Experience in Crisis Intervention Preferred
  • Ability to communicate effectively and compassionately with individuals and families dealing with a mental health related crisis.
  • Understands, communicates (both verbally and in writing), and collaborates effectively and with precision, given the time constraints of each task.
  • Ability to provide an Affidavit of Good Moral Character
  • Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.

LANGUAGE SKILLS: English (required); fluency in Spanish is preferred

JOB TYPE: Full-time (non-exempt) Monday through Friday from 9a-5p

*Fridays are conditionally remote with other remote day opportunities

Job Type: Full-time

Pay: $25.00 - $27.00 per hour
