Join Paving the Way Multi-Service Institute as a Case Manager, where you'll play a vital role in supporting individuals and families in need. This position offers an exciting opportunity to make a difference in the Washington, DC community while working with a dedicated team of professionals.

Responsibilities:

• Conduct comprehensive assessments to identify client needs and develop tailored service plans.
• Provide ongoing case management support, including regular follow-ups and progress monitoring.
• Collaborate with community resources and service providers to ensure comprehensive support.
• Facilitate access to services such as housing, healthcare, and employment assistance.
• Maintain accurate and up-to-date client records and documentation.
• Advocate for clients' rights and needs within the community and service systems.
• Participate in team meetings and case conferences to enhance service delivery.

Requirements:

• Bachelor's degree in social work, psychology, or a related field prefered .
• Minimum of 2 years of experience in case management or social services.
• Knowledge of community resources and social service systems in Washington, DC.
• Strong communication and interpersonal skills for effective client engagement.
• Ability to work independently and as part of a collaborative team.
• Proficient in case management software and documentation practices.
• Compassionate demeanor with a commitment to client advocacy.
